B. MAKING REGISTRATION FORM
Application Form and Handbook can be obtained by paying Rp. 15000.00 in the Admissions Committee Secretariat ISI Yogyakarta Rectorate Building, Jalan Parangtritis Km.6, 5 Sewon, Bantul. Telephone (0274) 379133, Fax 373 659 (0274) 371 233. Intake Form starting on March 29, 2010 or can be downloaded via ISI Yogyakarta Website: www.isi.ac.id
C. GENERAL TERMS AND CONDITIONS
1. Selection implementation costs USD 150,000.00 paid upon return of registration form. Additional costs for the implementation of selection programs Option Two studies in the Faculty of Arts of USD 75000.00.
2. Made two copies of the registration form and submitted to the Committee Secretariat
3. Statement of willingness to pay contributions Academic Quality Improvement (SPMA) and Semester Credit Unit Cost (SKS).
4. One copy of STTB / vocational diploma / high school equivalent and Nun that has been legalized or Certificate of Final Examination of the Principal concerned.
5. Four sheets of black and white passport photo size 3 × 4, seven pieces for those who also took the second option.
6. Income certificate of parents who have been approved by the Treasurer (for civil servants, TNI / Police, Private Employees) and to others approved by the local village chief.
7. Health certificate and free drugs from the doctor.
Note: Having declared acceptable, para 6 and 7 may be submitted at the time of registration.
D. SPECIAL TERMS AND CONDITIONS
In addition to the above requirements, candidates must also meet the following requirements:
1. Healthy lungs and throat, for the FSP all Programs.
2. No color blindness by a specialist unless Studies Program Art Music, Art Karawitan, and Ethnomusicology.
3. No physical disability, mute and stutter, to FSP Department of Music, dance, puppetry and theater that can interfere with the study process.
